Mafia: The Old Country is set in Sicily during the 1900s in and around the fictional town of San Celeste, which was previously featured in Mafia II. It is based around the origins of the Sicilian Mafia. The narrative follows Enzo Favara (Riccardo Frascari), who journeys from a childhood of indentured labor as a carusu in Sicily's sulfur mines to joining the crime family of Don Bernardo Torrisi (Johnny Santiago). The game received mixed reviews from critics, who praised the game's narrative, characters, Sicilian setting, visuals, and return to a linear structure, but criticized its static open world, dated gameplay mechanics, repetitive combat, and clichéd storytelling.

In Their Own Story

In 1904, 17-year-old Enzo Favara and his friend Gaetano are carusi working in the sulfur mines of Don Ruggero Spadaro. After a gas leak collapses part of the mine, burying Gaetano alive, Enzo scars Spadaro's underboss Damiano "Il Merlo" Bastoni in a knife fight and flees. He is rescued by Don Bernardo Torrisi and begins working for him under underboss Luca Trapani. Over the years, Enzo proves his loyalty by subbing in for a jockey in a horse race, rescuing the Don's daughter Isabella from bandits, and killing the traitor Girolamo Ludovici. He is inducted as a soldato and begins a secret relationship with Isabella. In 1907, war erupts between the Torrisi and Spadaro families, leading to the deaths of Don Galante and Luca. Enzo assassinates both Fontanella and Spadaro in Palermo, but becomes disillusioned as Torrisi grows mad with power. When Enzo tries to escape with a pregnant Isabella, he is confronted by Torrisi, mortally wounds him in a fight, and is fatally stabbed by Cesare. Isabella escapes, kills Tino, and boards a ship to Empire Bay, reading a letter from Enzo.

Reader's Guide

Mafia: The Old Country is a third-person linear, narrative-driven game set in cinematic environments. This brings the game more in alignment with Mafia and Mafia II rather than the more open-world oriented Mafia III. While primarily linear, the player can diverge from the primary path during certain story chapters to explore the environment, or sometimes to reach optional objectives. A DLC story expansion titled Man of Honor was released on August 14, 2026. In Man of Honor, set in December 1905, Enzo tracks down a thief stealing Torrisi's wine, then assists Ennio Salieri (Andrew Russell) in retrieving stolen money from two former associates, including a confrontation at a sulfur mine overseen by Alfio Zitelli.
